JACK FM’S $10,000 CELEBRITY LOOK ALIKE CONTEST

    1. To enter the $10,000 Celebrity Look Alike Contest starting at 12:00am PT on April 25, 2007, go to www.jackfm.com upload a photograph of yourself and the celebrity you look like, fully complete the submission form before 12:00pm PT on May 11, 2007, the Contest Closing Date. Only one entry per person.

      To submit an entry as a look alike without visiting our website, send a photo of yourself, along with a photo of the celebrity you look like, with your name, address, day/evening telephone number(s), date of birth (for verification purposes only) on a piece of paper and mail it with sufficient postage to 2440 Ash Street, Vancouver, BC V5Z 4J6 Attention: Celebrity Look Alike to be received on or before May 11, 2007. You will receive one (1) entry into the Contest during the entry period for which your request was postmarked and received. Limit of one (1) mail in request per postage stamped envelope per day.
    2. All submissions received before the Contest Closing Date will be posted on www.jackfm.com from 12:00am PT May 14, 2007 to 11:59pm June 10. Online voting for this contest will take place during this time period. Every day listeners will be able to vote for the top 20 contestants that they think most looks like the celebrity. Only one (1) vote per email address per day of voting . Listeners who register to vote will be entered into a draw to win 50,000 Jackaholic Club points and 2007 Ducati M695 with custom paint from Bike FX courtesy Richmond Motorsports Ducati (retail value $14,000 Cdn)
    3. The entrant which receives the most amount of votes each night between Monday and Thursday up until 11:59 pm will be announced the following weekday morning on 96.9 JACK FM, the entrant that has most votes up until Sunday 11:59 pm will be announced on the following Monday, until there are 20 (twenty) qualifiers. These qualifiers will be eligible for the grand prize of $10,000, and the first runner up prize, and second runner up prize.
    4. All 20 (twenty) qualifiers must be present at the grand finale event “The Event” to be held on Thursday evening, June 14th at a time and location in the Lower Mainland. A panel of judges at the event will decide the winner of JACK FM’s $10,000 Celebrity Look Alike Contest, and the first and second runner up prizes from the 20 (twenty) qualifiers.
    5. A random draw on June 14, 2007 for the listeners who participated in the voting will be made by a representative of Jack FM (“Rogers”) at the event. The selected entrant will be contacted via phone or email. If the winner cannot be reached by June 15, 2007, incorrectly answers the skill testing question, declines the prizes another entrant will be selected on or before June 15, 2007.
    6. No purchase, expenditure or investment is necessary to enter this contest
    7. If the identity of an entrant is disputed, the authorized account holder of the e-mail address submitted at the time of entry will be deemed to be the entrant. The individual assigned to the e-mail address for the domain associated with the submitted e-mail address is considered the authorized account holder. A selected entrant may be required to provide proof that he/she is the authorized account holder of the e-mail address associated with the selected entry. All entries must be submitted from a valid e-mail account that may be identified by reverse domain name search. The sole determinant of time for the purposes of receipt of a valid entry in this contest will be the contest server machine(s).
  1. (a) Celebrity look alike participants will be eligible to win one (1) grand prize of $10,000. The first runner up will win $3,000, and the second runner up will win $1,000.
    (b) Voting participants will be eligible to win 50,000 Jackaholic club points and a 2007 Ducati M695 with custom paint from Bike FX courtesy Richmond Motorsports Ducati (retail value $14,000 Cdn)
  2. All prizes must be accepted as awarded and may not be transferred or exchanged. Rogers reserves the right to substitute a prize of at least equal value in the event of the unavailability, for whatever reason, of the advertised prize.
  3. In order to win, selected entrants must first correctly answer, unaided, a time limited, mathematical skill testing question, posed by telephone at a mutually convenient time. Rogers’s judges' rulings are final and without appeal in all matters related to the promotion and the awarding of prizes.
  4. Selected entrants will be required to return a signed release and acknowledgment form before being declared a winner.
  5. By entering this contest, each entrant consents to the use of his/her entry, name, address and/or photograph in any publicity carried out by Rogers and/or its advertising agencies, without further compensation. Winners consent to their photograph being taken by Rogers for such use, without further compensation. All entries become the property of Rogers.
  6. This contest is open to residents of the Lower Mainland area (defined area includes Vancouver, West Vancouver, North Vancouver, Burnaby, New Westminster, Surrey, Coquitlam, Port Moody, Port Coquitlam, Pitt Meadows, Maple Ridge, White Rock, Abbotsford, Mission, Langley, Surrey, Delta,. White Rock, Richmond, Ladner) who have reached the age of majority except employees of Rogers, its parent and affiliate companies; its representatives and agents, the contest suppliers and those with whom they are domiciled.  The contest is subject to all applicable provincial and municipal laws and regulations.  Contestants in the look alike portion of this contest cannot be professional look alike personalities.  “Professional” means having ever represented an event as a celebrity look alike. 
  7. No correspondence will be entered into except with the selected entrants. To receive a copy of the contest rules or for the names of the winners, send a postage paid, self-addressed envelope to 2440 Ash Street, Vancouver, BC V5Z 4J6 Attention: Celebrity Look Alike on or before June 8, 2007.
  8. Chances of winning the voting portion depend on the number of entries. Rogers is unaware of how many entrants there may be. The winners may come from any geographic area, so long as they otherwise qualify to enter the contest.
  9. All entries which are incomplete, illegible, damaged, irregular, have been submitted through illicit means, or do not conform to or satisfy any condition of the rules may be disqualified by Rogers. Rogers takes no responsibility for lost, misdirected, late or destroyed entries, or for typographical or other production errors.
  10. All entries become property of Rogers who assumes no responsibility for lost, stolen, delayed, damaged or misdirected entries or for failure of the Internet or Rogers network during the promotional period, for any problems or technical malfunction of any telephone network or lines, cell sites, microwave dish, antenna, switch, computer on-line systems, servers, access providers, computer equipment, software, failure of any communication or traffic congestion on the Rogers or other network, or any combination thereof including any injury or damage to an entrant’s or any other person’s wireless device or personal computer related to or resulting from playing or downloading or sending any material in the promotion. Rogers reserves the right, in its sole discretion, to cancel or suspend the online or email portion of this contest should a virus, bug or other cause beyond the reasonable control of Rogers corrupt the security or proper administration of the contest. Any attempt to deliberately damage any equipment or system or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws, and should such an attempt be made, Rogers reserves the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution.

No further informational or marketing communications will be received by the entrant unless the entrant provides Rogers with explicit permission to do so at the time of entry.   12. This contest will be run in accordance with these rules, subject to amendment by Rogers. Entrants must comply with these rules, and will be deemed to have received and understood the rules if they participate in the contest.
  13. The terms of this contest, as set out in these rules, are not subject to amendment or counter-offer, except as set out herein.
